SCHOLARSHIP NAME: National ChairScholars Scholarship

AWARD AMOUNT: Up to $20,000 for tuition at any college or university of their choice. Fifteen to twenty new students are taken yearly and are given $3,000-5,000 per year for each college year to a maximum of $20,000 total

ELIBIBILITY CRITERIA:
- The student must be physically challenged and may be "chair-confined" but does not have to be.
- Must have serious financial difficulties.
- Could not go to college without this financial aid!
- Must have a history of satisfactory performance in the past. At least a B+ average must be proven!
- Must be a high school senior or college freshman. No graduate students can be considered! (No one over age 21 should apply--our "charter" does not permit it.)
- The applicant must show some form of major community service or social contribution in the past.
- No graduate study past four years of college can be funded.
- The physical challenge must be significant. No minor disability will be considered.
- ALL APPLICANTS WILL BE CONSIDERED WITHOUT REGARD TO SEX, RACE, or CREED.
SCHOLARSHIP NAME: Spina Bifida Association of America Scholarship Fund
ORGANIZATION: Spina Bifida Association of America
AWARD AMOUNT:
Lazof Family Foundation Scholarship – Applicant must be a high school junior/senior and graduate from a 4-year college or university within 4 years. Students will be granted $2,500 in tuition every year for 4 years.
SBAA Four-year scholarship – Applicant must be a high school junior/senior. Students will be granted up to $5,000 every year in tuition and room and board for 4 years.
SBAA One-year scholarship – Applicant must be high school graduate or possess a G.E.D. Students will be granted $2,000 for one year.

- Applicant must have spina bifida. A physician statement of disability is required, including the physician’s address and phone number.
- Applicant must graduate from a 4-year college or university within 4 years.
- Applicant must maintain a 3.0 GPA. Grades will be monitored to ensure that they meet the criteria.
- Applicant must maintain a full time class schedule throughout their college/university years.
- Lazof Family Foundation Scholarship recipients must complete their degrees in four years and make proportionate progress every term toward that end. The intended scholarship is paid at $2,500 for tuition at the end of each year if the student meets the criteria. Four scholarships will be awarded.
- SBAA Four-Year Scholarship Criteria:
- Applicant must have spina bifida. A physician statement of disability is required, including the physician’s address and phone number.
- Applicant must show financial need, using funds to attend a school that would otherwise be out of reach financially.
- Applicant must be an applicant of, enrolled in, or accepted by a college or university. A letter of acceptance is required. For applicants who have not yet been accepted by a school, final awarding of the scholarship is dependent upon acceptance.
- One scholarship will be awarded subject to funding availability.
- SBAA One-Year Scholarship Criteria:
- Applicant must have spina bifida. A physician statement of disability is required, including the physician’s address and phone number.
- Applicant must be an applicant of, enrolled in, or accepted by a junior college, approved trade, vocational, or business school. A letter of acceptance is required. For those applicants who have not yet been accepted, final awarding of the scholarship is dependent upon acceptance.
- Up to five $2,000 scholarships will be given.
