There is no formal application form. To be eligible for consideration, applicant must submit the following information:
  • Applicant's name and year in school
  • Home and school address of the applicant and telephone numbers
  • College major
  • College honors, offices and activities
  • Any other accomplishments or information the applicant would like to provide
  • A statement (one page minimum) signed by the applicant, telling how the scholarship would contribute toward the applicant's future career goals
  • The names, addresses and telephone numbers of at least two references
  • Recommendation letters from each of the references sent under separate cover directly to the scholarship chair
  • A certified copy of the applicant's transcript sent under separate cover directly to the scholarship chair
