New Employee Account Request Form

*** PLEASE NOTE: This form is only available from on-campus or via VPN when off-campus.  If you submit from off-campus while not connected to VPN, the submission will fail ***


This form should be used by supervisors, departmental admins and/or sponsors on-behalf of the person needing an account.

For NEW employees, this form may be used to request accounts network logins, email, and voice mail accounts.  It is also used to request re-assignment of phones and computers.

For EXISTING employees, is may be used to change phone and voice mail extensions.

For temporary visitors, interns, work-studies, etc this form can also be used to request new accounts.  Specific access needs and purpose of access must be specified in the Comments section. Also please note that liability of accounts created for non-employment/non-faculty positions may fall to the requester.

Please complete prior to the individual's first day on campus, preferably as early as possible. The Help Desk will create the account and send an eMail to the requester containing all needed information for accessing the new account(s). If you have any questions, please contact the Help Desk at extension: 4357.

A confirmation email will be sent to the requester's email address once the form is submitted.

Please Note: The name entered here will be used when creating accounts. If a nickname is preferred as part of their name in email and accounts, that version of the name should be entered here. Changing names after accounts are created requires deleting and rebuilding all components of the account.

Please be aware that although you are able to request a particular phone and voicemail extension, your choice cannot be guaranteed. Unless the extension being requested was previously assigned to your department, the new employee will likely receive the next available extension.

Desired Caller ID Display:

This is the name displayed on phones when placing calls from their office phone. It can be the employee's name, their position/title or office name. Please enter it exactly as you'd like it displayed.
