Team Captains, Please read and agree to the following in addition to the above statement.

Each team should have a team captain who is elected or appointed to be the spokes-person for the team. This person shall act as a liason between their team and the Intramural and Recreational Sports Program. Matters affecting a team are communicated to the team captain who in turn is responsible for informing his/her team members.

"As captain of the above named team I accept resonsibility for the team and the information to be given to all participants. This is to certify that I have read and comply with the Intramural Elegibility Rules and Risk Statement."

All rosters are final by the start of the playoffs. Any participant for a team must be on the respective team's roster. (New names may be added between 9am-4pm in the SAC Office)

Participants can only play for one team in each division(Men, Women, and Coed Divisions)

All participants must be students, faculty, or staff at USC Aiken. Spouses may be approved to play by the Director of Student Activities, or Student Coordinator of Intramural Sports and Staff.

***$10 forfeit fee is due when roster is turned in to Student Life Office
