Accident Reporting for USCA Students and Visitors to campus
The University of South Carolina Aiken’s Student and Visitor Accident/Injury Report form must be completed for every incident or accident occurring on campus by students or visitors. Any event that involves injury to a person or damage to property, or has the potential to do so, must be reported to the Environmental Health & Safety Manager within 24 hours.
Procedures for Reporting an Accident/Injury Involving a Student or Visitor:
- Seek medical attention immediately. For smaller emergencies, students can go the Student Health Center or Family MedCenters of Aiken. See address for Family MedCenters of AIken below.
- For immediate attention, call USCA Public Safety at 6111 or 648-4011 to report the event. USCA Public Safety can contact Aiken emergency dispatch as needed.
- For incidents occurring in a classroom or university related event, the instructor or person in charge should be notified of the incident immediately, and a Student & Visitor Accident Report Form must be completed within 24 hours of the accident and returned to the Environmental Health & Safety Manager. Incidents occurring at any other time or place on campus must also be reported within 24 hours to the Environmental Health & Safety Manager.
- Student & Visitor Accident Report forms can be obtained from the Department of Operations or printed from this website.
- An accident/injury occurring to students where insurance claims are filed, students should contact Trish McClain in the Student Life Office to complete the Special Risk Accident Claim Form for the University’s student health insurance policy.
