All participants in any activity or service offered by USCA must complete this form. If a participant is under 18, both the participant and their guardian must complete this release. If applicable, the participant and the guardian must sign this release in the presence of a witness. No participant will be permitted to participate in any outing or activity offered by USC Aiken unless and until this release has been completed.
