Application Process and Checklist
Eligibility
- Have a diagnosed intellectual or developmental disability
- Received special education services while attending K-12 schools (e.g., Individualized Education Plan)
- Have exited/completed high school
- Able to demonstrate flexibility and the ability to manage stress effectively
- Have enthusiasm and desire to live independently, including maintaining gainful employment
- Can independently manage own hygiene and medications
- Have the ability to communicate
The Process
Online application is NOW OPEN. Applications are due November 15, 2025.
There are several requirements to apply for USCA LIFE.
As part of the online application, you will submit the following information:
- Academic and employment history
- Please provide the names and contact information for three references. The references must have known the student for at least one (1) year and should represent at least three of the following: education, employment, community involvement, or personal. Family members should not complete the reference forms.
- Personal support inventory completed by parent/guardian
- Student questionnaire
- Release & Exchange of Information form
You will also upload the following documents in the Application Portal.
- Student’s most recent psychological evaluation (must list diagnosis and have information about intellectual functioning and adaptive skills)
- Student’s most recent IEP
- Student’s academic transcript (if applicable)
You will also be asked to pay the $45 application fee.
Application Checklist
- Complete the student application online.
- Pay the $45 application fee online.
- Complete the Personal Support (to be completed by parent/guardian).
- Complete the Student Questionnaire (to be completed by the student).
- Identify three (3) references using the Reference Form.
- Attach or request the student's academic transcripts from high school.
- Attach the student's most recent psychological evaluation.
- Attach the student's most recent Individualized Education Plan.
- Complete the Release & Exchange of Information Form
